Why the Same Estrogen Patch Dose Doesn't Mean the Same Thing for Everyone

Two women start the exact same transdermal estrogen patch, at the exact same dose, on the exact same day. One feels like herself again within weeks. The other still has hot flashes, still isn't sleeping, and wonders if hormone therapy just doesn't work for her. Both outcomes can happen on an identical prescription, and new clinical guidance on transdermal menopausal hormone therapy helps explain why.

What the research found
This is a clinical guidance piece, not a single trial, and it makes a fairly simple argument: there's no one estradiol dose or blood level that works for every woman, because skin absorption of transdermal estrogen varies enormously from person to person. Skin thickness, blood flow, body composition, patch or gel formulation, and even ambient temperature all change how much estradiol actually gets into the bloodstream.
That's not theoretical. A 2024 real-world study measured blood estradiol levels in 1,508 women using transdermal estrogen and found a wide range of results even among women using the same licensed dose [1]. One in four women on the highest approved dose still had levels low enough to be considered under-treated. Older women and patch users, specifically, were more likely to land in that under-treated range.
The clinical implication is straightforward: bone protection, symptom relief, and cardiovascular safety may each require a different serum estradiol range, and a standard starting dose is a reasonable first guess, not a guarantee. For some women, checking a blood level partway through treatment is a useful way to see whether the dose is actually landing where it needs to.
The bigger picture
This fits a broader shift in menopause care toward individualized dosing rather than fixed protocols. A 2025 narrative review found that both benefit and risk from hormone therapy depend heavily on timing, route, and dose, with transdermal estradiol at low-to-moderate doses generally preferred over oral estrogen when cardiovascular or clotting risk is a concern [2]. A large Medicare-based study of 10 million women found that outcomes varied meaningfully by route and dose too. Low-dose transdermal or vaginal preparations carried a more favorable risk profile than oral or higher-dose options across several outcomes, including cardiovascular events [3]. A 2018 systematic review covering more than 2.5 million women reached a similar conclusion: transdermal delivery, especially at lower doses combined with micronized progesterone, appeared to carry a lower stroke and clotting risk than oral formulations [4].
None of this settles into one universal number, and that's the honest state of the science. Effective symptom relief, meaningful bone protection, and favorable cardiovascular markers don't necessarily line up at the same blood level, and researchers are still refining exactly where those thresholds sit for different outcomes.
The naturopathic lens
Hormone dosing decisions belong to the prescribing clinician managing a patient's hormone therapy, and I stay in my lane on that. Where I spend my time is everything hormones interact with on the way in and out of the body: liver function, since the liver processes and clears estrogen; gut health, since certain gut bacteria (sometimes called the estrobolome) influence how much estrogen gets reabsorbed versus excreted; and stress physiology, since the adrenal and ovarian hormone systems are deeply linked.
This is why menopause care works best as a team effort. A prescriber manages the hormone piece. Whole-person care, the nutrition, digestion, sleep, stress, and liver support, shapes how well a woman's body uses whatever dose she's on. Neither piece replaces the other.
Conclusions to draw from this
If you're on transdermal hormone therapy and still have symptoms that haven't resolved, that's worth bringing back to your prescriber rather than assuming the treatment simply isn't for you. Absorption problems are common and often fixable with a dose or delivery-method adjustment, and a blood estradiol level can help clarify what's actually happening. If you're supporting your body's hormone metabolism more broadly, focus on the fundamentals: fiber and cruciferous vegetables for gut and liver support, consistent sleep, and stress management. Those won't replace a prescribed hormone dose, but they support the system that's processing it.
